Q. How do I begin a Medication Therapy Management review?
A. Your pharmacist will take you through a few easy steps:
- Complete and sign consent and privacy (HIPAA) forms that allow your pharmacist to obtain medical information from your physicians and other pharmacies where you might have prescriptions filled.
- Your pharmacist will collect your medical information and enter it into a secure software program that organizes your medical history. You may be asked to come into the pharmacy to review your medical history in person.
- Once the information is collected and organized, your pharmacist will go through the information with you and provide you with a report detailing where you may be able to improve your drug regimen. Your pharmacist can review the report with your physician to determine which recommendations are best for you.
Q. Can I see the medical history that my pharmacist has created?
A. Your pharmacist uses MTMPath software to organize your medical history, which creates your PMR Smart record. PRM Smart is accessible from any computer with an internet connection: to you, your pharmacist, your physician, or any one you designate. The correct information for your treatment is available to you anywhere, anytime. PMR Smart is safe and secure; no one but you can give permission to see your records.
Q. Can I continue to add information to my history?
A. Yes, you are encouraged to ask your physician for copies of your records and lab tests every time you have an appointment. Bring these records to your pharmacists to have them added to your medical history so that it is kept up to date.
Q. Can I enter information in my medical history?
A. If you monitor your own vital signs at home, including your blood pressure, weight, or blood glucose, you will be able to track these on your home computer using your secure PMR Smart internet account. Your home monitoring data will be available to your selected healthcare provider when you go for an office visit.
